Birth Certificate
Required
Issued by the Registrar of Births and Deaths, Srinagar Municipal Corporation, or any other competent authority.
Child’s Aadhaar Card
Required
Issued by The Unique Identification Authority of India (UIDAI).
Photographs
Required
Recent photo of child and parents or guardian (if any)
Identification Proof
Required
Aadhaar Card of both parents.
Residential Proof
Required
Any one of the following: Voter ID, Passport, Electricity / Water Bill.
Child’s Immunization Card
Required
Issued by concerned Hospital or Government department.
Child’s Medical Fitness Certificate
Required
Issued by a MBBS cerrtified doctor.
Legal Dispute Documentation
(if applicable)
Relevant court documents or orders if there is an ongoing legal matter.
Single Parent Documentation
(if applicable)
Proof that: One parent is no longer alive, or There has been an irrevocable separation with no second marriage. An affidavit is required for verification.
